Many serious accidents have happened because a machine was started when it was supposed to be completely shut off for maintenance or repair. These incidents are typically the result of the energy being not released properly. Lock-out-tag-out (LOTO), which ensures that machines are completely off until a person with authorization releases them, helps to prevent these kinds of accidents.
The LOTO system is made up of a number of locks, tags and other devices used to secure, isolate, or block a machine from electricity. All "affected", "authorized" and "qualified" personnel should be instructed in the use of lockout/tagout devices. These devices include, but are not limited, to chains, locks, and padlocks. Each device is unique and labelled, including the date and time that it was put in use along with the name and title of the person authorized, and the purpose of the lockout/tagout (repairs, maintenance and so on.). It should also be clearly stated that these devices are not able to be removed until an entire LOTO procedure has been followed.
